An international drivers license is simply a document that translates the information on a license to multiple languages and certifies that the document is indeed a driver's license of a particular class. In most countries the international license is prepared by an auto club. It carries no authority as a license itself, so we get back to whether or not the government in question accepts the foreign license.
In China and Barbados I had to get a local driver's license. In China I had to surrender my license issued by a U.S. state, to get that license, and had to take a road test.
For someone coming to the U.S., it depends on the state you are driving in, and sometimes the country in which the license was issued. Most states are pretty liberal about this, if the license can be read and understood for what it is. That's where the international license comes in, to verify that it is a driver's license and not just an identity card.
