I put mine on the outside and used 90 degree fittings. That turned out to be a mistake because I still have to work around the tire. I have a concrete surface and it would have been much easier to use a creeper and had a straight shot at the zerks. If you have a decent surface and don't mind crawling under, I suggest putting them inside.
The grease holes often wear in and seal off on the bolt making it impossible to force grease in without jacking by the frame. The holes should be towards the area where the greatest load is on the bushing, but that makes greasing even harder. To remedy this, carve a figure 8 inside the bushing before installing. You can use something like a Dremel tool, just don't go very deep as to weaken the already too thin bushing.
Lynnmor, on the Dexter kit I just purchased they show installing the bolts so the grease holes are on the sides of the bolts or horizontal so that should have prevented the issue your having getting the bolts to take grease.
