This is a bogus excuse. I am a retired postal employee, and there are procedures in place to handle "hold" mail and "forward" mail. When the carrier has an order to hold mail, they are supposed to put a plastic block in the carriers case to prevent your mail from being placed in that slot. If there is a problem here it sounds like the regular carrier is not placing the block in the case. Blaming it on the substitute carrier is wrong, but a convenient excuse I suppose.
Unfortunately, mail forwarding (and mail service in general) varies quite a bit from one Post Office to another. Usually the smaller Post Offices give better service, but not always. As I said earlier, there are good procedures in place to handle your mail, but procedures don't matter if they are not carried out by employees. I would complain over and over to the Postmaster until they got it right. Sometimes it is better to make your complaints in writing to the Postmaster, and keep a record for yourself. If it doesn't get fixed in a reasonable time, contact your District office.
