In our city, whether the tree is on public right of way, utility easement, or private property, tree maintenance is the responsibility of the adjacent proprty owner. On residential streets, the specified clearance for overhanging traffic lanes is 12 feet, which is enough for the large vehicles expected to be on those streets: waste removal, street cleaners, school buses, parcel delivery, moving vans and rentals, fire trucks, ambulances. Vehicles that don't fit these clearances are supposed to stay on the streets and roads designated for large vehicle traffic.
Like most of our property maintenance codes, this one is enforced on the basis of complaints.
We do not require permits for tree trimming, if the trimmer is licensed to perform that service in the city. DIY is not usually a problem, unless you generate a complaint by damaging someone elses property.
Main roads outside residential areas, maintenance might be city, county, state highway departments. Budgets and staffs are inadequate, so action usually requires a complaint.
I don't think it a reasonable expectation that some government is responsible to make sure that every road and street is passable to any size vehicle someone wants to bring through. There are other issues besides trees, like lane widths, turning clearances, lighting and traffic control equipment, that mean some places simply won't be suitable for me to take my RV, and I venture into those places at my own risk.
