Yes, at least I did with my 6" frame. Here's a photo - new style head, raised ball, 6" frame. Maybe the main clearance issue was where the chain attaches to the trailer? I'm not sure. I just recall I needed to put the raised ball on for some reason....

Kurt sells one. Maybe your guy can get their stuff? I could only get a "raw" one. I just cleaned it really well and then sprayed it with couple coats of clear lacquer paint to keep it from rusting.

Yes, the bar is centered on the cam. The snap up chain should be as straight as possible. You can see from these wear marks on a pretty new cam where the bar rides:

There are black painted carriage bolts in the bottom two holes (3rd from last photo from JBarca above). I'm not sure if he did the top hole too? If you do, you need to ensure the chain does not hit when the snap ups are up. Also, be aware that the top hole might be in line with the pinch bolt.
