Too many unknown factors to even guess. Was the driver experienced in towing? Speed? Condition of brakes? TT and TV loading? Was it a panic situation? Did the driver fall asleep? DUI? Any number of, or combination of several small things at the wrong time could cause this. Drive like this could happen to you and you'll be better off for it.
