All of the above. Yes, it does look like the chain is on the wrong hook. Bars should be parallel to the trailer frame. My hitch requires not dropping more than 3 links off the hook. This allows enough chain travel that it doesn't bind when turning. You will probably have to pick up a considerable amount of weight off the truck using the tongue jack to get the chains secured. Get a cheater bar about 18" long. 1/2" pipe works good, available at any hardware store.
