Since everybody is speculating, I'll throw in my speculation.
Along the lines of your "HitchitSafe" my guess is the guy forgot or did not put a padlock on. The trailer bounced off the ball and broke free
X10 happens to the best of them.
I had one of my friends who tows cargo trailers everyday for a living.
I had him pick up a truck for me from Montana and bring it back to Chicago on a flat bed trailers.
When we unloaded the car the trailer popped right off the ball! He drove all those miles with it either not locked down correctly or it bounced open, BUT he also did forget to put the lock thru it.
His face went completely white thinking of all what could have happened on the road versus it happening parked in our lot like it did.
So yup it can happen.
