Couple things to getting #3 under control.
You need tension between the 2 to "stretch them straight again". Period. Either hammering trailer brakes while maintaining speed.....OR and this is risky, drop the hammer on the tow rig and pull that sum beatch back straight again.
This can or sometimes must be down along with driving with the sway. Over correction, all with more minor than say bringing a vehicle out of a skid will exaggerate what the trailer is already trying to do to kill you. IF you have the room and you need to know wt f you are doing, you use the whole road and drive with the sway, getting the 2 more parallel or inline before having to come back the other direction.
These are the 3 things one can do to combat #3 and win.
Oh and cause #4. You're overloaded or trailer is loaded bad of whatever causes the immediate wiggles at a certain speed....drive just under that speed. Yes you have a poorly set up system, but it doesn't manifest into bad sway until that pendulum can gather enough momentum. Many miles pulling bad loads and you gotta sit at 56mph or some slow speed to keep er rubber side down.
