Backing at too sharp an angle between TV and TT is a likely cause of the Frame Bracket slippage (yoke doesn't "slip" -- frame bracket slips and allows rear end of yoke to move).
The theoretical maximum articulation is about 83 degrees. If you jackknife the TV/TT into a sharper angle, something is likely to give.
Frame Bracket slippage is one of the possibilities.
The PP installation instructions should specify a correct amount of U-bolt nut torque.
