Probably caused by lack of support below. My floor looked like they used a chainsaw to clear for the plumbing. You should have the plumbing checked for cracks as well. My drain pipes were cracked because they were supporting the weight. This stuff is pieced together by folks that don't even have plumbing in their own houses.
