I use ladders nearly every day. The worst combination is a guy afraid of heights and not trained to set up the ladder correctly. If I have a new hire that I see hanging onto the ladder for dear life, he is relegated to ground duty only or maybe a 6' step ladder on occasion. Accidents happen quickly and it isn't worth the risk of someone that dislikes heights getting on a ladder. I've seen my wife actually freeze up from fear on an extension ladder.
I use a name brand telescoping ladder for the RV when out camping. It extends 12' which is 2' above the roof. That allows for safe travel on and off the roof.
