The employer would be required to report the value of what is received as income. Depending upon the situation, it would either be on a W-2 or a 1099.
Yes, of course, although that is not for the OP to worry about.
It would be something for them to worry about since it would be clear evidence the OP was working in the US and when they entered the country they clearly affirm they are not coming into the US to work, know they cannot work unless they have a green card and know that a violation of that affirmation could lead to them being asked to leave the country and never return.
