Straight extensions do not change the torque setting. Using anything that changes the distance from the handle to the center point of the fastener will, such as a crowfoot or a universal (swivel) socket.
If you want to be able to properly torque lug nuts (generally 100+ ft-lbs) as well as smaller fasteners, probably two torque wrenches are in your future.
Agreed, you probably want a 3/8" drive and 1/2" drive if you do a lot of automotive work. If you're only buying 1 and just need it for lug nuts, get a 1/2" drive.
