Based upon the label it seems those testers are nothing more than 3 neon bulbs connected as follows:
Red: Between Neutral and GND
Middle Yellow: Between Neutral and Hot
Right Yellow: Between Hot and GND
If you look at the chart all of the scenarios make sense for this simple setup.
Since neon bulb AC testers work fine at 120V or 240V, my conclusion is that all 3 lights would be lit if a 120V receptacle is wired up for 240V and the ground is connected. The center yellow will be powered by 240V and the red and right yellow will be powered by 120V.
If the bulbs cannot tollerate 240V, then only the red and right yellow ones will be lit.
