Wired Wrong, not cuz it has 2 breakers cuz someone can't see the markings on the receptacle for w/white (neutral) and g/green ('ground').
You want to blame it on 2 breakers while its the other end that is wired WRONG. The other end is color coded!!!

You should not use a dual pole breaker for camper/RV setups by default. You should use a single pole 110/120v breaker. Had I had the correct breaker, it would have been fine. It was not the wiring.
Technically, if you wire a 240v dual pole to only put out 110/120v, it will fail inspection.
