I didn't realize they can just fail. FWIW, I do recall some whistling noise from the valve in the past when the flush was running; just assumed it was normal. Maybe it was a sign of impending failure?
Either way I'll start by replacing it and see if that fixes it.

You may not have found a loose connection but if the flush valve failed it would simply be clogged and that clog would be inside the tank which would not cause a leak. If it is clogged and you are seeing water in the coach you need to find out where the water is coming from. If the flush valve is broken at an external connection, that could also be the source of the water but it will be external to the flush hardware.
