I trust (fervently) that Executive's coach uses LUGS on the Rear Wheels. Here's a pic of a LUG:

There are Studs coming out from the Hub. Inner Dual goes over the Studs. Then Lugs are threaded over the Studs. The raised Flange at the bottom of this pic bears on the Wheel's mounting holes. Lug is torqued using the square head. Wheel is now solidly in place. This is what allowed removal of the outer wheel to fix the tire.
Then... center portion of Lug (between Square down to the Flange) is threaded. A Nut for the Outer Dual is threaded onto that part of the Lug and torqued.
So, if the coach uses 10-hole wheels, the duals are torqued 20 times each. 10 on the Square for the Inner wheel and 10 on the Nuts for the Outer.
