Before we got out residential refer we fought with keeping the temp below 40. What seemed to work best was the ice pack swap method. Freeze the ice packs at night and move them to the refer while driving. The ice packs we used are "Techniice HDR Dry Ice Sheet" (http://www.techniiceusa.com/ice-packs.html). because they are sheets rather than some bulky shape you can lay them on top of your food for better cooling.
Also turning on the ice maker is going to make the unit work harder, leave it off. I ended up removing mine for more freezer room.
