I saw those same numbers in the Norcold owner's manual that someone else quoted. However, I've been on roads with steeper grades than that. Does that mean my fridge will die as soon as I drive up a hill with the fridge running?
RV manufacturers do not include levels, it is up to the owner of the RV to add a level if you so wish. Personally I can not imagine anyone willing wanting to camp out of level, just way to uncomfortable.
As far as between the lines goes, standard levels generally used in carpentry has a glass vial that has a colored liquid which is not filled totally full (this allows for a "bubble" to form).
The vial is not straight across but has a slight curve to it with the middle being higher than the ends.
These levels are calibrated in such a way that it is level when the bubble is centered between the line markings.

As far as driving with fridge on, in theory, no, should not hurt since you are in motion and that will keep the ammonia solution moving around..
The problem starts when you stop moving for long periods of time (hrs, days, weeks and so on), should be OK for short stops like fueling, eating or sight seeing..
On edit.. Forgot to mention, you want one level on the front of the trailer (shows side to side level) and one on the SIDE of the trailer near the front (shows front to back level.
You can get a round "bulls eye" level which you can place in the fridge (typically bottom shelf of fridge).
Once you have the trailer level according to the bulls eye level in the fridge you can mount your outside levels so they will indicate when the FRIDGE (and therefor the trailer) is level.
Levels ARE CHEAP, RV FRIDGES, not so much.
