Internal short. My take, - it was hit while being loaded in the bin. I know a guy that had a fire when his dog tried to chew small 18650 LiPo cell. Like in atomic bomb, 2 parts that should be kept separate, get in contact and start reaction.
Making more resilient casing would be against this culture of disposable bottom-quality items.
