Most computer fans do much better with resistance in front of the impeller, rather than behind it.
In this case of both Noctua fans blowing inward on case lid, well there was likely not enough area for air to exit.
Really with the location of the MW's temp sensors which control the 40Mm fan, any one orientation could keep 90% of the internals cooler than another, but the sensor might be in a dead zone, airflow wise.
With both fans blowing inward, when the 40Mm fan would kick on, its flow, and the heatsinks, simple did not feel hot.
An IR thermal imager would be a much better tool for judging efficacy of any particular fan orientation, but that will not be happening.
