John, your diagnosis is correct. There is one tab missing and it looks like it's been missing quite a while or maybe it was worn so much that when the awning hung up, it came out then.

This is on the left side of the slide near the back of the rig on the door side. The pictures before and now are oriented right. As for the other tabs, both right side tabs have little wear. The left side tab on the right arm is worn considerably down to maybe a 16th of an inch. It stills seems to be working. There is only a small dime-sized metal on metal mark about midway down the inner arm on that side. That mark is shiney as opposed to rusted like in the the picture. That might indicate that the rust in the picture has been there since the awning incident.
Are these tabs nylon?
So with both the previous picture oriented right, both racks are tracking left. I looked at the tabs on the opposing slide and they look good. But on that one both racks are tracking on the outside of the pinion gear--i.e., to the left on the left assembly and the right on the right assembly.
I believe you mentiond four tabs per arm. Where are the other tabs?
As you say, the plot thickens. I have a feeling I'm going to know a lot about slides before this is over. ;)
