Our Motorhome came with the screw on valve extensions. For the most part they work fine. A few years ago, I had a problem where one of the inner tires won't take air and if I tried to check the pressure the pressure would be 0. Like you, I took the extension off and blew air into it and it worked fine. Put it back on and it still did not work.
Our valve extensions have a jam nut which increases or reduces the amount of thread on the extension on both ends. On the end that attaches to the valve stem, I tightened the jam nut about 2 complete revolutions and then they worked much better. I haven't had a problem since.
It is possible there could be a problem with the valve core or the valve stem. I had a valve stem on a previous vehicle where the valve core would rattle loose. You can buy a valve core removal tool at any auto parts store for a buck or two. Remove the valve core and re-seat it.
