For a while, when the 6.7 Cummins was new (07 to 09 primarily I believe), The turbos were manufactured with a cleaning port to take care of this problem. Today, not so much. At least that's from the Cummins POV. Seems like a procedure was even posted for the cleaning of the exhaust brake:
