Yes any good fab shop would be able whatever you want
That said... You should think about turning a standard rack into a 3-point setup. Then, you can simply bolt or weld on two more receivers to the receiver side-plates (Reese sells weld-on receiver tubes) and then bolt an extra pair of 2" square tubes to the rack.
I would not worry about welding on the front hitch. The bolts are plenty strong, and because of the dissimilar wall thickness between the frame and receiver side-plates, it would be difficult (but not impossible) to get a good weld in there.
