Your front disk brakes are doing ALL the work. The rears needed to be adjusted manually even tho they should be self adjusting. Your rear pads are most likely glazed and shot!
I tend to agree with cummins.... Perhaps the proportioning valve is not functioning correctly.

How many miles do you get on the front pads?
On my 98 I removed the proportioning valve and wa-la the rear brakes actually started working. With rear antilock brakes it was a great mod.
