The transmissions already exist. It is just a matter of being willing to install it and market it. There's plenty of demand for it, people just don't want derated trucks just to have a manual. A manual that would hold up would sell and did sell in the past.
Unfortunately for those that want it, the market has already told all the manufacturers that the demand just isn't there. I used to far prefer manual transmissions, but automatics have so far outstripped them in efficiency and driveability that I'd not buy a manual.
