Just when did the 1st crew cabs come out ?
I didn't think they were around back in 1964, thought it was more like the 1980's. Anyway here's a nice short story of a retro D200 Super charged 5.7 Hemi to boot.

International Harvester introduced the first crew cab in 1957. It had 3 doors; the 4th door was added in 1961.
Dodge in 1963.
I think the forest service used a lot of the early crew cabs as well.
