One of my big problems on the 6.0 powerstroke has been the Fuel Injection Control Module (FICM). I went through two of them in 20,000 miles at $800 a piece. Detuning the engine doesn't do anything to help the electronics that are used to control it.
Most of the rest of the issues on my engine didn't start showing up until it was near 100,000 miles. At that point a lot of them were related to the emissions. I had a series of cascading failures after the EGR cooler got clogged. No coolant going through the cooler caused the cooler to melt allowing coolant to enter the combustion chambers and blow a head gasket. The clogged EGR cooler also prevented oil flow through the engine oil cooler.
I'm hoping that Ford has addressed these issues in their new engines.
