Ma dually pickup owners - reg question?

Ok, so I am looking at buying a dually pickup for personal use and from what I am reading in Ma. I will be required to get a commercial plate. possibly pay about $300 a year for the reg and then a DOT inspection? will I need to provide a medical card also? this will not be registered to a business, but to me personally.
