The HDT guys figure if you're going the MDT route, you may as well go all the way. My biggest problem with the HDT was the paucity of automatic transmissions. No stick for me. That, and the fact I don't care if I can climb a mountain at 65mph.
You're not current on HDTs!!
HDTs have been available with semi-automatic trannys "Autoshift" (driver must "clutch' at start and stop), and full automatic tranys Freedomline & Ultrashifts for a long time!
The newest automatics - Ultrashifts and Volvo's I-shift (showing up on used HDTs) have awesome features.
Due to the fuel savings for fleets (and surprisingly lower maintenance) - the "row your own" preference by some OTR drivers are becoming a moot issue.

BTW - the mtn at 65 mph is a moot issue also.
The bigger engines are just loafing along on flat or hills.
Resulting in excellent MPG for towing *RV* trailers.
Three stage Jake coming *down* the mtn is WAY nicer than the power going *UP*.
Possible to never touch the service brakes.
