When brake shoe moves cable is pulled and star wheel gets turned.
That keeps brakes adjusted as shoe material wears down.
With cars the cable only would pull and tighten the brake when you stopped while going in reverse. My dad used to back up in the yard quickly and hit the brakes a few times a couple of times a year to keep them adjusted tight.
