I'd personally go with the ultimate package. I love my sunroof, in unfortunately this truck didn't have it and I really miss it. Never came close to hitting it with my kayaks on my old truck. Built in navigation is nice, usually a bigger screen and better sound on the train by turn directions.
PTO, not really sure you'd need that on a truck, use my tractors every weekend. Wheel liners to me not needed but a good set of mud flaps are. Tailgate step and fifth wheel package a definite yes.
