With the latest, been a while now, updates the 6.7 Cummins of 07.5-09 is just as reliable as 10-12, considering the hard parts (aside from minor changes to the HO in 11.5) are the same it was just tuning changes to improve the emissions reliability.
As far as a manual trans goes the Cummins is your only option, based on your accurate research of Ford and GM dropping the manual with the LBZ.
I agree. Many people have not had their 7.5 to 09 Cummins 6.7 reflashed with the newest program, which does take care of most of the early emissions issues. The 6.7 CTD on my 08 Ram 2500 with over 99K miles has been perfect. It's a towing beast.
If you are considering the Ford 6.7, you might want to do some internet research. They are having some problems and Ford is trying to deny warranty coverage saying it's caused by bad fuel. It is a small percentage, but there are some angry Ford 6.7 owners on the Ford forums. Spend some time there reading.
A Duramax is also a good choice if you don't like the Ram interiors. I really like the Ford's but I think it's 6.7, while way better than the 6.0 or 6.4, is the least reliable of the 3 based on my reading. JMHO. YMMV. Good luck.
