That said, if the QC and failsafes are so great, then how do any bad ones get through?
Something out of process happens, IE a part has to be replaced because it was found to be faulty in a downstream test process. Usually repaired in an offline repair area without failsafes, rely on the repair person. The main reason for a "bad one" is a defective part from a supplier. The current Ram waterpump recall is a good example.
