I discovered it was the camper sagging under the weight of the mattress and us, the sleepers up there. The bed area has mostly thin framing/ structure, so you'll get some bed sag to the center in lots of these campers.
We folded towels to various thicknesses and laid them under the mattress for a straight bed. This helps a lot but, takes time to get it right. - bigfootfordNomad II

- Deb_and_Ed_MExplorer III ordered a 4" thick, dense foam mattress topper from Amazon, then trimmed to fit. If I remember correctly, you want foam that's 2.5 lbs per sq ft or heavier if you're using it as a mattress?

These can be trimmed to fit any size... The additional three inches just touches the POPUP ceiling when lowered down for travel.

Make sure your rv floorplan allows you remove a mattress easily by bending it. Its sometimes easier to tie or strap it in half lengthwise to remove it and install the new one.
Pay attention to the thickness of the new one so it doesn't interfere with a cabinet or wardrobe door from opening.

I'm looking for a short queen mattress myself right now and having trouble finding one with a height of 5" as most of them are 8" or 10" since I need to find one that size for cab-over clearance so I don't knock myself out while trying to sit up in bed.
