It will eventually have a failure in the EGR cooler, which will cause the oil cooler to fail, which will cause a failure in the fuel injectors, which will ultimately lead to a head gasket failure.. which will cost you new head gaskets and 5k to repair.
Ok, was that shocking? I hope so, because that is very much a possible reality for you if you buy this truck. For Many Many Many people who "this would never happen to" it has happened in spectacular fashion.
Now are you willing to invest 5k either proactively (bulletproofing) the motor or if you are prepared to shell it out at a future date, that is fine. The 6.0 is a strong and well driving motor, but if you choose to drive one of these you must be prepared for this kind of fairly common failure.
A 6.0 original "barn find" like what you have found is not quite like finding an original 1970 Chevelle SS in a barn. A lot of the 6.0's with many many many miles have already had a lot of this work done to them.

Well it is past 100,000 miles zero warranty. How really cheap is he willing to sell it to you for? Is the price low enough that you can invest another $1,500 to $5,000 for a partial to full bullet proofing, for some peace of mind.
Myself, I feel just fine taking off on any trip with my 01 Ram with 262,000 miles on it.
