The GM tow/haul mode is best for city/urban hauling of heavy loads. Basically it locks the transmission into shifting at specific rpm points. My Chevy Express (6.0L, 6 speed trans) upshifts at about 2100 rpm and downshifts at about 1700 rpm, so it goes into overdrive (6th) at about 65 mph. If I were at near capacity I would use it but normally I only use it if the transmission starts "hunting" excessively. Since I usually tow in the low 60's, not knowing this, I would never get into the top gear and my mpg would suffer accordingly.
