Slider alone won't do it, as you will hit the sides of the box when you turn. The only option is a regular hitch with the Reese airborne sidewinder pin box.
This will move the pivot point both up above the height of the bed and back about 30 inches while keeping the weight forward on the hitch.
Might even go so far as to say this is your only option unless someone else makes a similar hitch design.
edit: I must have been into the sauce when I typed this - I assumed you had a ram box for some reason.
