According to the terms on their website, NY does not list any maintenance fees of any sort if you have your statements delivered by email and have the account linked to a credit card. You do have to maintain a floating balance in the account, but I don't see any indication that it goes away over time. (They do also have a link-to-checking-account variant that doesn't keep a floating balance.)
Note that this is different than getting it through the NY/NJ Port Authority, which does charge maintenance fees.
I think there are other states that also have no annual or monthly fees, at least with certain plans or payment setups.
