FMEA sometimes find odd / interesting things.
A certain aerospace vehicle had 4 identical computers with a totally different backup computer designed, built and engineered by a different team.
On its maiden "drop test" flight where it was released from the back of another aircraft, all 4 computers (except the 5th) failed simultaneously, leaving the vehicle to land on the backup.
FMEA revealed that the fault was in a particular soldiered joint, which was overstressed, and as it did so, it came apart.
Now, that is one of the best engineered systems, and with software that is reckoned to be the most thoroughly checked / debugged in the world. Still, current consensus is that there is 1 undiscovered bug per 10,000 lines of source code even as the system is being decommissioned after a run of 4 decades.
What I am involved in now is to get a handle on this phenomena, and deal with the technical problems caused by systems that are now so complex that it is reaching the limits of even large teams to grasp and debug, and to mitigate the FMs using a fundamentally different set of design philosophies.
The question is at what point does it... get too big for us --- or do we still have a ways to go in developing technologies to grasp the issue?
