This shows (if to scale) the very or too small cavity for lubing
the cam/followers.
Add that if the 'inlet' to the cavity and the 'outlet' to the cylinder
intake too close to have the rest of the cam/follower cavity stagnant
with 'poor' lube property diesel and you have a very, very poorly lubed
cam/follower which has very high PSI conditions
Then add the other two variables that seems to coincide with failures
#1 H2O and #2 Heat
Both reduce lubricity
Both then creates a better environment for cavitation.
