I gave him a call and he sent me the link to the form you have to fill out and said it has to be mailed with a police report on the accident with the VIN number of the vehicle on it. The form has the address.
FORMIn your case the police report may not be possible but it would not hurt to try filling out the form & mailing it in anyway. Maybe a report from the insurance company?
Other solution if the TIP was registered in your name is to put the new one in your wifes.
BTW the line in Spanish "Por lo cual anexo la siguiente documentación" means "I am attaching the following documents". That implies they wants some sort of documentation.
