The form that Tequila's link goes to is the same one that I have on my website; only mine also has an English translation. I also tell you what documentation you need and how to handle all this (Example: do NOT use the mail system!).

Oh, and TIP stands for Temporary Import Permit. I also describe where and when you need one of these things on both locations. You'll also find out what RCNC stands for :B Which appears to apply to your situation!
