speaking of miswiring, hot neutrals, the cheap testers etc. years back I read an article in pop Science or one of those about surge protectors etc. and at that time they were using MOVs and most all only had the hot wire protected and they had a diagram putting MOVs between all of the various pairs of wires. I made my own, added their feature to isolate an outlet to plug in a printer as that could cause interference or something, and plugged in my $5.00 3 LED tester to check my work. to make life easy I plugged in a standard 6 plug outlet strip to plug my newly devised surge protector into.
the LED tester showed that I had reversed the two leads so I looked into it and no I had done it right, checked my wall outlet and that was correct so I checked the 6 outlet strip and found out that that had been manufactured incorrectly.
I notified my friend who at that time worked at Product Safety and they came out and confiscated (well they did pay me for them) the two strips that I had bought and found out where I had purchased them.
So even a manufacturer can have a screw up at times.
