I agree with you. Different locations different results. In Troncones a few years ago wife dropped a sink cover on foot, swelled up huge and badly bruised. In pain I took her to the nearest hospital in Zihuat, what a 3rd world place. She was taken to an emergency room and the sheets on the bed had blood on them. Dr came in and after consultation sent her down the hall to the x-ray room, walking. There were halls that appeared to be under construction, paint peeling,plaster falling. Got the x-ray walked back to dirty emergency room. Dr came showed us the x-ray, slight fracture. Rapped the foot and prescribed medication. total cost to us for RX and emergency including x-ray under $30.00 U.S.
The outcome was good for the issue she had, but I would never imagine being sent there for a heart or stroke issue. On the other hand have seen some pretty clean hospitals in Maz and P.V.
Now in P.V. there is a Vet that has a better facility than that hospital in Z.
