- I flip the loop on cable over hitch handle
- If the hitch flies out of the bed of the truck, you have worse problems than worrying about the breakaway.
I catch mine up in the lock on the safety latch on a Reese hitch. - I use a carabineer and hook to the bed rails
- I connected my cable to the truck bed near the edge of the tailgate with a carabiner. Just make sure the cable can't get tangled up with anything during manuevering.

We hook it to a dedicated loop that was installed on the bumper. - I put an eye on my hitch so I could hook the brake switch wire to the hitch. Been doing that for 18 years now. I don't think it makes much difference where you hook it as long as it hooked to the tow vehicle.
