I keep destroying my secondary regulator. Help

I would suspect a failed pigtail or primary regulator that is backfeeding the high pressure regulator.

The high pressure regulator is required by code because the connecting hose exceeds 60".

What exactly is the failure? Have you tried a different brand? I like the Cavagna regulators and pigtails.
