I was all set to order new when I found that the rail heights are about the same as the overhang height of my 2005 Cardinal. I still haven't decided whether to fix up the old truck body wise or get a new truck and change to a flat bed. I am thinking of fixing the old as the wife is starting to complain about getting into the old so there is no way she would be happy getting up into the newer trucks.
For 15 years we pulled with an 05 Dodge 2500 two wheel drive. When my new F250 2WD came in I put a new B&W hitch in and guessed at the bolt hole settings. It actually came out about the same as the old truck!!
No height or clearance problems and the 5th wheel sit level. Just stay away from 4WD and you should be fine on new trucks.
